1. Information involved in our services
When you contact us, book, or receive services, information may include your name and contact details, appointment information, payment and membership records, mobile-session logistics, intake and consent responses, and professional session notes.
We do not receive your answers from the short Executive Stress Audit on this website. If you choose to share an observation or result during intake or a session, it can become part of the information you provide for that service. The earlier Jotform audit is a separate submission process.
Please keep health history and other sensitive details out of general email or text inquiries. Use the intake process provided for your appointment.
2. How information is used
Information supports responding to inquiries, arranging appointments, reviewing service fit and consent, preparing mobile sessions, delivering and documenting care, administering payments and memberships, and providing appropriate service follow-up.
Information should be limited to what is needed for these purposes. An ESA result is educational context, not a diagnosis.
3. The check, booking, and intake
The short Executive Stress Audit on this website processes answers within the page. It does not send those answers to Eustress AVL, put them in a link, save them to browser storage, or add the visitor to a mailing list. Refreshing the page clears the answers. This is separate from the earlier Jotform audit and the appointment intake system.
Booking links open Acuity Scheduling. The Eustress AVL Acuity account is connected to Square for appointment payments. The earlier audit is hosted by Jotform. Those services process submitted information under their own policies and the settings of the Eustress AVL account. Payment and communication providers also process information needed for their functions.
Intake and session records
Client intake and consent are completed through Eustress AVL’s intake website. The intake application is hosted on Render and stores submitted information and related session records in its database. Practitioner access supports reviewing intake, preparing sessions, documenting care, and appropriate follow-up.
Practitioner AI assistance
The intake system supports practitioner-initiated AI assistance through OpenAI. The application removes structured identity fields and checks selected context for identifying details before a request. Submitting an intake does not automatically trigger an AI request. AI-assisted material requires practitioner review and does not replace professional judgment.
Removing identifying fields does not guarantee that all context is anonymous. Contact us before your session if you have questions about this capability, the information used, or your preferences. This website notice does not itself give consent to process your session information with AI.
4. Website operation
This replacement website does not include a shopping cart, customer account, embedded intake form, location-permission request, advertising pixel, or site-authored analytics cookie. Images, styles, the short self-assessment, and the downloadable PDF guide are served with the website. Downloading the guide does not require an email address or form submission. Booking and intake links take you to separate services.
This website is hosted through OpenAI Sites. Hosting providers may process technical request information, such as IP address, browser details, and request time, to deliver and protect the website. This is separate from the short check’s answers, which stay in the page. See OpenAI’s privacy policy for its handling of information.
5. Service messages and marketing choices
Appointment coordination and service follow-up are separate from permission to receive promotional messages. Completing an audit or booking a session should not be treated as automatic marketing permission.
The short check and direct PDF download do not require an email address. If you use the separate ESA guide-request form, Jotform collects the email address you enter, your optional first name, your marketing choice, the request time, and the form’s source and consent-copy version. Jotform sends the requested guide as a PDF email attachment. This request does not include your assessment answers.
Ongoing recovery emails are optional and use a separate, initially unchecked checkbox. Jotform sends guide requests to a private connection on this website to check that choice. Only opted-in requests are forwarded to Klaviyo, with the email address, optional first name, and consent/source details. Klaviyo asks you to confirm before joining the recovery-email list. The guide remains available whether or not you confirm. You can unsubscribe using the link in ongoing emails.
The form opens in a separate tab when you choose it; its service may process technical information needed to deliver and protect the form. See Jotform’s privacy policy and Klaviyo’s privacy notice. Guide requests and marketing contacts are separate from clinical intake and session records.
You may contact us about communication preferences and use the unsubscribe instructions in promotional messages. If an earlier signup or guide request did not work as expected, contact us for help.
6. Access and sharing
Access should be limited to people and providers who need information to perform their role. Marketing use of client stories, names, employers, photographs, or identifiable details requires appropriate permission. Eustress AVL does not sell personal information.
Information may also need to be retained or disclosed for applicable professional, legal, insurance, or accounting requirements.
7. Retention, security, and requests
Eustress AVL aims to limit access and use appropriate safeguards. No system can guarantee complete security.
North Carolina’s massage and bodywork rules require client files to be retained for at least four years after the client–therapist relationship ends. Other applicable obligations may require longer retention. Requests to access, correct, or delete information are reviewed in light of those obligations.
Retention depends on the type of record and the applicable obligations. This notice does not promise automatic deletion after a fixed number of days or immediate removal from provider backups. Contact us to discuss a particular record or information request.
To ask about your information or request a correction, contact hello@eustressavl.com. Some records may need to be retained under applicable requirements; deletion should not be promised before the relevant record is reviewed.
